Alleles of the apolipoprotein E 4 (ApoE4) have been regarded as a risk factor of Alzheimer's disease. Though volume reduction in medial temporal regions have been reported in adults carriers of ApoE4, the effects of ApoE4 on healthy children remain unclear. The aim of this study was to examine the influence of ApoE4 over childhood. We compared regional gray matter volumes of E4 carriers with those of E3 (wild type of APOE) carriers in 181 children and adolescents (63 males and 81 females) aged 8-21 using MRI. No differences in age and gender between E4 (N = 37) and E3 (N = 144) carriers. T1-weighted MR images were analyzed using the Statistical Parametric Mapping 8 (SPM8), and voxel-based morphometry (VBM) analysis was conducted using the diffeomorphic anatomical registration through exponentiated lie algebra (DARTEL) procedures with 8-mm smoothing. Age, gender and intracranial volume were used as covariates of no interest. We detected volume reduction in the left posterior hippocampus and the right parahippocampal gyurs (P <.001 uncorrected) in healthy children with E4 compared to E3 carriers. Those medial temporal reductions in E4 were significant with small volume correction (P <.05, FWE corrected for peak level). Our results suggest ApoE4 can induce reduction in medial temporal regions even in healthy children.
