Cardiovascular autonomic function in patients with epilepsy

Heart rate variability (HRV) is a useful tool for the detection of sympathetic-parasympathetic balance of autonomic nervous system in both normal and patients. Autonomic nervous system involvement in patients with epilepsy has rarely studied and has shown conflicting results. The aim of the study was to evaluate cardiac autonomic function and somatic sensation in patients with epilepsy. The study was conducted on 50 epilepsy patients who had tonic-clonic seizures comparing with 50 controls. A short term analysis of HRV for both the time domain and frequency domain measures; and vibration perception threshold (VPT) for peripheral neuropathy were assessed in both the groups. In time domain measures, the square root of the mean of the sum of the squares of differences between adjacent RR intervals (RMSSD) [46.4 (29.8 - 52.2) vs. 29.7 (26 - 34.5) ms, p=0.023] and percentage of consecutive RR intervals that differ by more than 50 ms (pNN50) [26.1 (6.6 - 37.2) vs. 10.7 (5.5 - 12.7) %, p=0.025] were significantly higher in patients. In frequency domain measures, high frequency (HF) [114.5 (74.5-179) vs 182.5 (104.25-247) ms2, P=0.006] was significantly low in patients; and low frequency (LF) [141 (104.25-249.75) vs 115.5 (83.75-140.75) ms2, P=0.021], and LF/HF [56.8 (43.02-69.17) vs 33.3 (24.52-53.22) %, P=0.002] were significantly higher in patients. VPT was comparable between the groups. These data suggest that epilepsy patients have an impact on the cardiac autonomic function as measured by HRV.
